Challenge
Following serious safety incidents and a federal investigation, a multinational refinery needed major systemic improvements. Progress remained inadequate after several years, and a subsequent audit required completion within one year to protect its right to operate.
Response
PMO Partners and plant management defined seven workstreams, integrated their plans, and established decision-making, progress monitoring, accountability, and communications. The effort covered safety documentation, operating limits, mechanical integrity, asset records, preventive maintenance, protective systems, and employee training.
Outcome
Each workstream achieved its defined objectives within the one-year timeframe. The source reports that meeting the federal mandates allowed the refinery to continue operating. The engagement supported a stronger safety operating model; it does not establish a claim that all future incidents were eliminated.
